New log in instructions:

  1. Navigate to the practice Patient Portal login page.
  2. Select ‘Login’.
  3. Enter the email address on file for the patient.
  4. Retrieve the six-digit one-time passcode sent to the email address.

  5. Enter the one-time passcode to complete login.

To access, click the button below to be directed to the portal.  Your login will be the email address we have on file for you.  A six-digit one-time passcode will be sent to your email and then you will enter the one-time passcode to complete login.

CareCredit can be used to cover the cost of both the eyecare and eyewear you need to see your best.  Items that can be paid for with CareCredit include vision exams, glasses, sunglasses, contact lenses and dry eye treatments.
